Home visiting service

Castle Partnership manage the Central Norwich Home Visiting Service – a team who visit 70,000 patients across Central Norwich. Our Team consist of Advanced Paramedic and Nurse Practitioners – who are supported by the Practice GPs to provide care to our housebound patients when they require it. We also provide home visits for long term and chronic disease reviews to support our patients in managing their conditions and maintaining their health at home.

Advanced Nurse Practitioners

Clare Sullivan – Care Home Clinician

Clare has been a Nurse for over 30 years, and an Advanced Nurse Practitioner for 15 years. Clare’s early career was spent at the old Norfolk & Norwich hospital in General Surgery and ITU. She has worked at the Walk In Centre and for Out of Hours and now concentrates mainly on supporting the Care Homes within the Central Norwich Primary Care Network. Clare enjoys walking and dining out with her husband and socialising with friends.

Jack Duggan

Jack qualified as a Paramedic from the University of Greenwich in 2018 and spent 5 years working for East of England Ambulance Service. Jack works at both Holt Medical Practice, and Castle Partnership. Jack has a keen interest in palliative and end of life care as well as dermatology. Outside of work Jack can often be found in one of Norwich’s excellent pubs!

Adam Tiffany

Adam started his career in healthcare working in histopathology in England and in Australia before joining the East of England Ambulance Service in 2016. Adam recently completed his pgCert in Urgent Care and works still for the Ambulance Service, alongside his role at Castle Partnership. Adam enjoys being active and visiting as many countries he can.
