Accessing your health record

When you visit an NHS or social care service, information about you and the care you receive is recorded and stored in a health and care record.

This is so people caring for you can make the best decisions about your care.

The information in your records can include your:

  • name, age and address
  • health conditions
  • treatments and medicines
  • allergies and past reactions to medicines
  • tests, scans and X-ray results
  • specialist care, such as maternity or mental health
  • lifestyle information, such as whether you smoke or drink
  • hospital admission and discharge information

How to get your GP record

Your GP record includes information like any conditions or allergies you have and any medicine you’re taking.

Most patients will automatically be given access to more information added to their GP record. This includes letters, test results and appointment notes. Some people may also have access to information added to their GP record before this date. You can access your GP record and nominate someone you trust to access it too.

There are 3 main ways to get your GP record.

  • using your NHS APP account: log in or create an account
  • Register for our online services
    • upload or visit the surgery with photo ID so we can confirm your identity
    • if you are setting up access for the first time, we will give you a username and password
    • go to SystmOnline or the app for access
  • using other online services or apps: GP online services and apps

Access to other medical records

For any other medical records, you need to ask for them at the NHS service you went to. This includes records like your hospital record.
