- are registered at the surgery
- have been invited to do so
If the information given has an impact on your contraceptive, you may need to attend a face to face appointment.
No method of contraception is 100% reliable and have to be taken properly to achieve the NHS quoted reliability rates.
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
