Social Prescriber & Care Coordinators

Our Team of Social Prescribers and and Care Coordinators support our patients in many ways. If you are struggling with bereavement, loneliness, hoarding, struggling to care for a loved one, struggling with self-care, need support with accessing financial support or anything else that is not a clinical need – they are there to help.

Our Care Coordinators ensure patients are followed up for chronic disease reviews, learning disability reviews, routine vaccinations and more. They also support our Care Homes to ensure that their residents/our patients receive the very best care.

If you are needing support for symptoms of ADHD or autism – our Care Coordinators will support you through this process and to access diagnosis where appropriate.

Emma Edwards – Social Prescriber & Care Coordinator Lead

Emma worked for OneNorwich Practices as their Primary Care Network Care Coordinator doing various roles within different practices and working as their Admin Lead for the Vaccination Team for housebound patients within the PCN. Emma chose to be a Care Coordinator because personalised care within the healthcare system is important to her. Since joining Castle Emma was given the opportunity to really bring a personal approach to patients, first as a Care Coordinator and now as Social Prescriber and Care Coordinator Lead. Emma now manages a whole personalised care team.

Emily Beare – Care Coordinator & Practice Learning Difficulties Champion

Having worked in various roles in other surgeries as well as a brief time at the N&N as an HCA Emily really enjoys being a care coordinator. She enjoys the focus of personalised care and getting to know our patients and how best to support them. This support can vary from support with filling in forms to supporting patients holistically throughout their cancer diagnosis and treatment.
